At what mileage should we, do we, need we, use an additive.

If they act as a system cleaner can they be used from say 30K miles.

Or do the costs balance out if Premium grade diesel is used. Will it do the same thing.

I reckon that question is wide open to debate! I think and have seen from experience that more problems are likely if the diesel engine is only used for lightly loaded, short journeys. They really need to be grafting and running at working temperature to avoid carbon build up, bore glazing, turbo fouling etc.
If you're running a common rail diesel I'd be inclined to just use premium diesel to hopefully lubricate the pump a bit better than low sulphur diesel.
Personally on my old 1.9pd, I chuck in the odd tank of premium diesel as it does seem to run smoother and give slightly better mpg, but I don't sweat if a tank of tesco goes in. I have used a strong mix of redex additive in an old David Brown tractor with bad injectors and it did clear them, but would rather just use fancy diesel in the car. Some guys here swear by a drop of 2 stroke oil in the diesel

From David Brown tractors to combines to cars , diesel engines just need clean fuel and clean air.

They also need to decoke the exhaust, so to myself its get engine up to operating temp, put the car in 2nd gear and blast it for a few hundred yards, keeping it in 2nd gear and the revs just under the red.

Replace your fuel filters every 20K or at least drain any water out of them, you may find it easier just to replace the filter.

I just put in supermarket fuel & I don't have any problems.

As for the David Brown injectors, when I was in agriculture college, we were advised to change the injectors every 6 months and treat them like spark plugs. However no farmer does, and with the increased chance of water & debris in the fuel tank, even more reason to change the fuel filter
