Is it a genuine disc ?
Try running a lens cleaner.
If still no good remove the unit take the top off and clean the lens with a cotton bud.

Make sure it's genuine, RNSE doesn't particularly like copied discs as they run on DVD-Rom.

Weak signal is normally a failed amp. Above the rear screen. Expensive to replace.
A scan would help wirh all the problems you seem to be having.

Scan first!

Above the rear screen is a piece if plastic trim, pull that down and the amp is on the nearside.
The amp failing is a known fault.
On your nav try selecting the aerial to local, it might improve reception.
As Chris says your first move should always be a scan.
