The common reason is the double ended stud N 909 055 02 which costs £1.48, being installed incorrectly. It MUST be set into the head with the radius of the transition from M10 to M8 set below the tensioner mounting surface of the head or the tensioner stud bore outer edge will sit on the radius and cause premature or even instant failure.
