Auxilliary belt tensioner problem!

Hi, We replaced the auxilliary belt and tensioner on my son's Y reg Passat TDI 130bhp and since then the 100mm M8 bolt holding the tensioner in place has sheared off TWICE. Are we missing something or doing something wrong when putting the tensioner on.

There is to much tension on the bolt and the tensioner.
Suspect the alternator free wheel clutch is the cause.
Or the tensioner dampener has siezed

We will be taking a look at it Monday evening. What do I need to look for on the alternator pulley clutch?

jumping belt at idle on full lock and excess noise on full lock.
